As a guide, the practice serves the whole of Cheadle as well as areas of Cheadle Hulme, Cheadle Heath, Gatley, Edgeley and East Didsbury.

However, from 5 January 2015, all GP practices in England are free to register new patients who live outside their practice boundary area. This means that you are able to join practices in more convenient locations, such as a practice near your work, closer to your child’s school or even re-join your existing practice when you move home.

If you choose to register with Cheadle Medical Practice and you live outside our practice area we may not be able to provide home visits when you are not well enough to attend the practice.

If you would like to register with our practice, please download and complete the registration form and return it the reception Registration form pdf.  The practice leaflet and patient questionnaire can be accessed using the following link welcome letter & registration pack. Please return the questionnaire to reception along with your registration form.  

Registration packs are also available from reception.

Patients over 5 years old will be required to have a new patient medical with our health care assistant in order to be fully registered with the practice.  The health care assistant will take your medical history, medication details, blood pressure, height and weight and refer you to the doctor or nurse if necessary.  Medication will not be issued to patients until they have seen a doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.
